A. In the Present Perfect Passive, negation is formed with has/have not been + V3. General questions are formed with Has/Have + subject + been + V3.

In the Past Perfect Passive, negation is formed with had not been + V3. General questions are formed with Had + subject + been + V3.

B. The answers depend on the pictures. The things that have not been done yet should be described in the Present Perfect Passive form: has/have not been done.
